Should you wish to cancel any pre-booked appointment please ensure that you do give me 24hrs notice.
If 24hrs notice is not provided or if you forget to turn up to your scheduled appointment you will be liable for the missed appointment & charged full amount.
I will accept any cancellation that is left on my voicemail as long as they have been left 24hrs in advance.
At my discretion if I can fill your late cancelled slot then there is no fee.
Accepted forms of payment
I prefer cash, I accept payment by personal cheque and I can receive payment by Internet banking if the transaction is immediate and can be verified.
Consent to perform treatment
Before first treatment and at initial assesment I ask all my clients to sign a standard practitioner consent to treat form. The scope of the data collected is held securely by Sports Remedial and will not be diseminated to anyone but myself as a sole-practitioner. The data collected is a contact detail naming the client undergoing treatment and a set of medical history questions that would be pertinant to discuss prior to engaging into a course of manual therapy at clinic.
Unsolicited marketting
I run a busy clinic. Be warned if you are a cold caller who has harvested my number and is not a prospective client. Your will be reported to the telephone preference service and could risk a heft fine.

Non electronically stored data collected at point of purchase
If you are booking a firm appointment I will record a contact telephone number. When you visit the clinic for the first time and before treatment I ask all clients to complete a clinic sheet which does enquire about existing medical conditions and full contact details. These are not electronically copied, are confidentially filed and this manual copy can be destroyed upon request.
